Waiver Form for Hep C Meds

In October 2023, MedQUEST finalized a provider attestation form that waives prior authorizations for hep C medications, as long as:

1. Drugs are on the planʻs formulary;

2. Provider attests to using AASLD guidance, and has completed any Hep C education (eg, free CE module from University of Washington)
